Participant Information Form

The Participant Information Form is distributed to all participants when they first begin with TAP. The contents of this form are included within every virtual group Eventbrite registration. Participants only need to complete this form once. If a participant’s information changes, they can complete the form again. Every participant has the choice to opt out of completing this form.

Media Release Form

The Media Release Form is distributed to all participants when they first begin with TAP. This form is included within every virtual group Eventbrite registration. Participants only need to complete this form once. If a participant’s preferences change, they can complete the form again. Every participant has the choice to opt out of completing this form. Participants under 17 must complete this form with a parent/guardian present.

Confidential Information Release Form*

The Confidential Information Release Form is *only distributed when a parent/guardian or third party has requested the confidential information of a participant.

All participant information is confidential. Participant (or parent/guardian if participant is under 16 years of age) must give consent in order for participant information to be shared with a parent/guardian or third party professional.

Mandated Reporter requirements indicate that consent is NOT needed in the instance of a reportable case for a participant under 18 years old 1) where there is danger of imminent harm to self or others, and 2) in the case of apparent child abuse.

This form is to be completed by a member of TAP Staff in the presence of the participant (or the participant AND parent/guardian if participant is under 16 years of age). All information entered is to be verbally verified correct by the participant (or the participant AND parent/guardian if participant is under 16 years of age).
